In the case when there is no identifiable cause behind the problem, the treatment plan is usually based on the evidence-based treatments that have been proven to be effective in improving the fertility [4].

Generally, the treatment plan can include a wide variety of options. Doctors usually recommend options with fewer side effects for the first line of the treatment. But it can be different when the cause of the problem is very aggressive that can harm the overall health of patient. Again, the cause of the problem plays a key role!

Fortunately, most of infertility problem in men (90 %) is harmless and they often respond with the appropriate treatment [2]. Only about 1 % of all the cases that need serious medical attention because the problem comes due to serious medical conditions such as cancer /tumor, serious infections, or serious hormonal problems!

What next if the male infertility cannot be reversed?

If the fertility problems in men require medical intervention, the treatment plan can include [5]:

  1. The use of certain supplements (particularly if the cause is malnutrition or specific nutrient deficiency).
  2. Medications to improve intercourse problems or other problems associated with male fertility.
  3. Surgery! For instance, surgery is needed when there are anatomic abnormalities in the male reproductive organs.
  4. The use of antibiotics to treat a bacterial infection that affects the male reproductive system.
  5. Etc.

And when the problem doesn’t respond with the treatments mentioned above or when the underlying condition behind the problem cannot be cured, the use of ART or Assisted Reproductive Technology can be recommended.

In general, there are 3 major options to obtain sperm for ART procedure. It can involve obtaining the male ‘good sperm quality’ through surgical extraction, or normal ejaculation, or from donor individuals.

Then with medical procedure the sperm will be delivered to the female partner’s egg for fertilization. There are several choices for this procedure; by inserting the sperm into the genital tract of the female partner, or with intracytoplasmic sperm injection, or with in-vitro fertilization.
